Meet the Expertは、神経学のスペシャリストである講師と参加者が直接交流し、Interactive discussionを行うプログラムです。講師は、多発性硬化症、てんかん、パーキンソン病、アルツハイマー病などの分野でご活躍されている先生です。参加者は「45歳以下の神経内科医・後期研修医」30名です。講演・討論は英語です。留学を考えている若手研究員・大学院生、専門分野の選択を悩んでいる若手神経内科医、海外の神経学の実際を学びたい若手神経内科医、神経内科に入局を考えている後期研修医など、積極的な参加と討論をお願いします。
各セッションは定員制で、参加には事前登録が必要です(45歳以下の神経内科医、後期研修医に限ります)。

- Ranum教授はSCA1、SCA5、SCA8、DM2の原因遺伝子を同定し、C9orf72-linked FTD/ALSの分子病態としても注目されているRAN translationの発見者です。Paulson教授はMJD/SCA3における神経細胞核内封入体を最初に発見し、ポリグルタミンの神経毒性とMJD/SCA3モデルを用いた治療研究を推進しています。本企画では世界をリードする2名のマイクロサテライト・リピート病の研究者をお迎えして、本研究領域の今後の方向性や研究の魅力などについてお話をいただいた後に、参加者全員で討論を行いたいと思います。本領域に興味があり、積極的な質問や討論を希望する方々の参加をお待ちしています。

- Case discussion with experts provides us with the best opportunity to know how to solve the clinical question by leaning from the knowledge and experience of the specialists. Since epilepsy case discussion contains multidisciplinary clinical information (clinical history, EEG, MRI, neuroimaging, seizure demonstration by video, neuropsychology battery, pharmacological data, etc), it also makes your own knowledge both more practical and scientific.
Upon 2 case presentation from the institutes in Japan, we will cordially invite Prof. Samuel Wiebe (University of Calgary, Canada) and Prof. Byung-In Lee (Yonsei University, Korea) for their expert comments and will interact with audience in this session.

- 2名の世界的な臨床てんかん学のエキスパートをお招きしてのcase discussionを企画しました。
Samuel Wiebe教授は、現在国際抗てんかん連盟(ILAE)の理事で、多数例でのRCTでてんかん外科の有効性を初めて示した研究(N Engl J Med 2001; 345:311-318)を始め、神経内科の立場から多くの多施設共同研究を進められてきた第一人者です。
Byung-In Lee教授は、現在ILAEのアジアオセアニア領域の代表理事で、神経内科の立場から抗てんかん薬でのてんかん治療をはじめ多くの臨床研究があり、特に発作時SPECTの発案者です(Arch. Neurol. 1988:45:97-402)。

Title: Meet the Expert - On amyloid plaques, Lewy bodies, and prion rods: New perspectives on the role of amyloid in neurodegeneration

- Amyloid deposits in humans were described as early as 1639. Since then, prevailing hypotheses of disease causation have centered on these macroscopic protein deposits. Amyloids are key histopathologic components in many neurodegenerative diseases, including Alzheimer's, Parkinson's, and Creutzfeldt-Jakob. I describe here how our understanding of the etiology of amyloidoses in the nervous system has shifted away from amyloid fibrils and plaques to protein oligomers. I examine the increasing evidence for prion-like spread of disease pathogenesis. I end with a brief discussion of the roles and responsibilities of the neurologist in efforts to understand and treat dementing illnesses.

- アルツハイマー病(Alzheimer’s disease AD)の根本的予防・治療薬の開発は、高齢化が急速に進むわが国において緊要の課題である。これまでの知見より、AD発症の原因物質の一つとして、アミロ イドβ蛋白が疑われている。近年、ADをはじめとする蛋白コンフォメーション病において早期凝集体の存在（オリゴマー仮説）が注目されているが、 Teplow教授は、世界に先駆けて早期凝集体の存在を報告している。今回、オリゴマーの本態と形成過程を中心に、Teplow教授とともに有意義な討論 をしたいと考える。

Title: Meet the Expert - Diagnosis and Treatment of Early Parkinson disease (PD)
- Short summary

Identifying the ‚pre-clinical’ phase of PD is crucial for any disease-modifying intervention to delay disease onset and progression. Currently, no drug has yet been convincingly shown to slow PD progression, although data from clinical trials with MAO-B inhibitors have produced some, albeit insufficient evidence for such effects.
Once clinically diagnosed, there are currently three major classes of PD drugs with established efficacy and safety as initial monotherapy for early PD. These include MAO-B inhibitors, dopamine agonists and L-Dopa. Their effect sizes on motor symptoms as assessed by the UPDRS differ. Which drug to use as initial PD therapy remains a highly individualized decision weighing both drug-related and patient-related considerations. This presentation will review the available evidence and its practical implications for initial monotherapy in PD.

- オーストリアInnsbruck大学の神経内科教授 Poewe先生は、同大学で研修医を修了し、ロンドンのUniversity College and Middlesex Hospital Medical Schoolで主任研究員、Innsbruck大学で上級講師を務められた後、Virchow Hospital of Free University of Berlinにおいて教授を務められた。主にパーキンソン病とジストニアの臨床薬理学の研究をされており、これまでに共同執筆を含め550報以上の運動疾患に関する論文を執筆されている。2000年から2年間にわたりInternational Movement Disorders Society(MDS)の理事長を務められ、現在は日本神経学会の名誉会員に就いておられるなど、国際的に第一線で活躍されている著名な先生である。

Title: Meet the Expert - Differential diagnosis of multiple sclerosis and neuromyelitis optica
- Short summary
- Neuromyelitis optica (NMO) and multiple sclerosis (MS) are distinct diseases with different pathogenesis and treatment, but clinical and radiological findings overlap. No single finding is pathognomonic. Aquaporin-4 directed antibodies (AQP4-IgG) are highly specific, but depending on the assay, may yield false positive results. Furthermore, the sensitivity of AQP4-IgG is about 70%. The most useful indicators of NMO are longitudinally extensive T2-weighted lesions in the spinal cord at the time of acute myelitis and AQP4-IgG detection. Other clues (normal MRI at first presentation, prominent CSF pleocytosis or neutrophilic pleocytosis, absence of oligoclonal bands in CSF and certain characteristic brain syndromes or MRI lesions) may strongly suggest or support a diagnosis of NMO over MS.

NMOの研究者としても臨床家としても第一人者である米国Mayo ClinicのWeinshenker先生をお迎えし、主にNMOの診断にスポットを当て、我々が日常診療で悩んでいる問題点について御意見を伺います。まずは、苦慮することが多いMSとの鑑別診断について講演していただきます。
続いて、seronegative NMOとの相違、膠原病とのoverlap、さらにはOSMSについて、愛媛大学の越智先生を交えて、パネルディスカッション形式で討議を進めてゆきます。
